Cognitive Behaviour Therapy for Tinnitus in Nottingham | Oto App Relief

If you are one of the estimated 1 in 7 adults in the UK living with tinnitus, you know all too well that it is rarely just about a ringing or buzzing sound in your ears. It is about how that sound impacts your sleep, your focus, and your peace of mind. When people ask us for the most effective way to manage bothersome tinnitus, our answer is always rooted in clinical evidence. That usually points directly to Cognitive Behavioural Therapy (CBT).

Oto CBT tinnitus therapy at Ear Care Specialists

While CBT does not cure or "silence" tinnitus, it changes how your brain processes the sound. It guides you toward habituation—the psychological process where your brain learns to classify the ringing as background noise (like a humming fridge) and stops reacting to it with stress or anxiety.

The catch? Accessing face-to-face CBT on the NHS can mean facing painfully long waiting lists (if even at all possible in your local area), and private psychologist led therapy can be incredibly costly. That is why digital therapeutics like the Oto app are completely changing the landscape of tinnitus management both here in the UK as well as internationally.

What is the Oto App?

Oto is a specialized smartphone application designed by a team of doctors, audiologists, and tinnitus experts. It robustly packages the core strategies of traditional, face-to-face tinnitus therapy into daily, bite-sized audio sessions. Instead of just playing white noise to mask the sound, Oto combines targeted CBT techniques, mindfulness exercises, physical stretches, and patient education to help you reclaim control over your day.

What Does the Science Say? (The Clinical Evidence)

It is easy for any mobile app to claim it helps with health conditions, but Oto is one of the few digital tools that has been put through rigorous, independent academic testing. Two major randomized controlled trials (RCTs) prove its clinical effectiveness.

1. The Cambridge DEFINE Trial

Led by researchers at the University of Cambridge, the DEFINE trial was a large UK-based study involving 198 participants. It compared the Oto app head-to-head against the gold standard: one-to-one, therapist-delivered CBT sessions.

*The Result:** The trial demonstrated that smartphone-delivered CBT using Oto was non-inferior to face-to-face therapy.

*The Impact:** Roughly 64% of participants achieved a clinically meaningful improvement in their tinnitus distress within six months just by using the app.

2. The Flinders University Study (2025)

Published in the peer-reviewed journal Speech, Language and Hearing, this independent trial tracked 96 adults dealing with chronic tinnitus over several months.

*The Result:** Participants using Oto saw a statistically significant reduction in their Tinnitus Functional Index (TFI) scores—a standard questionnaire used by audiologists to measure tinnitus severity.

*The Impact:** Oto users experienced notable decreases in tinnitus distress by the 6-month mark, and these improvements were sustained at 9 months, showing long-term psychological retraining.

| Clinical Trial | Study Size | Core Finding | Key Metrics |

DEFINE Trial (Cambridge) | 198 participants | As effective as face-to-face therapist care | 64% saw meaningful distress reduction

Flinders University (2025) | 96 participants | Statistically significant, lasting relief | Relief sustained at 9 months

Did you know? Because of this strong clinical track record, Oto has been supported by the NHS Innovation Accelerator (NIA), mapping a clearer pathway for primary care clinics and GPs to offer immediate digital support to patients.

How Oto Guides You Through Recovery

Oto does not expect you to navigate your tinnitus alone. It breaks down the cognitive retraining process into an easy-to-follow digital roadmap:

1. Foundation & Education

   Week 1

Learn the science behind tinnitus. Understanding why your brain is hyper-focusing on the sound takes away its power and de-escalates the initial panic response.

 2. Cognitive Reframing

   Weeks 2-3

Begin targeted CBT exercises. You will learn to identify automatic negative thoughts about your tinnitus and gently rewire your emotional response to those triggers.

 3. Building Physical & Mental Strength

   Weeks 4-5

Incorporate mindfulness, breathing techniques, and physical neck/shoulder stretches designed to release physical tension that often worsens tinnitus perception.

 4. Deepening Control

   Week 6 and beyond

Solidify your tools for difficult moments, improve your sleep hygiene with the built-in sound library, and step into long-term habituation where the sound naturally fades into the background.

Taking the Next Step

Tinnitus can make you feel isolated, but the evidence is clear: you do not simply have to "just live with it." Digital tools like Oto mean that expert, clinically proven care is available the moment you open your phone. As audiologists, we see digital therapeutics as an incredible companion to your regular hearing care. Whether used alongside hearing aids or as a standalone tool, it puts the power of tinnitus rehabilitation firmly back in your hands.

On a personal note

I have now overseen the implementation of Oto CBT therapy for many of my tinnitus patients. I use to deliver similar (self-prepared) materials and exercises through face to face sessions. The day I discovered Oto and spent many hours going through the core materials and daily listening exercises myself, I decided it far superior to anything I could deliver face to face. In that moment, I knew I had to get it for my clinic; Ear Care Specialists, Nottingham.

The Oto CBT app (12 month license) is provided through our clinic, following an initial tinnitus assessment with Dr. Jeff | Audiologist and tinnitus specialist. We will also arrange a mid and end point follow up session to monitor and discuss your progress.
