Hyperacusis: Understanding Sound Sensitivity and Why It Happens

Hyperacusis is a condition where everyday sounds feel uncomfortably loud, sharp, or even painful. For some people, the clatter of cutlery, a passing lorry, or a colleague’s laughter can trigger a surge of discomfort that feels far out of proportion to the actual sound level. Although hyperacusis is less common than tinnitus, it can have a significant impact on daily life, affecting work, social activities, and emotional wellbeing.
In the UK, hyperacusis is recognised within audiology and ENT services, but it remains a condition surrounded by unanswered questions. Much of what we understand comes from clinical experience and emerging research — including work from organisations such as Hyperacusis Research — yet there is still no single explanation for why hyperacusis develops or why it affects people so differently.
What Is Hyperacusis?
Hyperacusis is generally described as reduced tolerance to everyday sounds. People often say that noises feel “too loud”, “sharp”, “intrusive”, or “painful”, even when those sounds are at levels most people find comfortable. Some individuals experience a sense of sensory overload, while others describe a distinct physical pain in response to sound.
Hyperacusis can occur on its own, but it frequently appears alongside tinnitus, hearing loss, migraine, autism spectrum conditions, anxiety, or sensory processing differences. For some, it develops gradually; for others, it follows noise exposure, illness, or a period of stress.
What Causes Hyperacusis?
This is where the science becomes more complicated — and where honesty matters. Despite ongoing research, there is currently no universally accepted explanation for the underlying mechanisms of hyperacusis.
Several theories exist, but none fully explain the condition:

Some researchers believe the brain may “turn up the volume” internally when it receives reduced or altered sound input, making everyday noises feel exaggerated.
Others suggest changes in nerve
activity or sensory processing may play a role.
There is growing interest in the relationship between the auditory system and the brain’s emotional centres, particularly in cases where sound triggers pain or fear.
Hyperacusis Research highlights the possibility of abnormal neural signalling in pain hyperacusis, where certain sounds activate pathways associated with discomfort.
Despite these ideas, no single test can identify the cause, and many people have no clear medical explanation. This uncertainty can be frustrating, but it does not mean the condition is dangerous — only that the science is still evolving.
Living With Hyperacusis
Hyperacusis can be distressing, not only because of the sound sensitivity itself but also because of the fear and unpredictability it creates. Many people begin avoiding noisy environments, wearing earplugs excessively, or withdrawing from social situations. While understandable, this avoidance can sometimes make sound sensitivity worse over time, as the auditory system becomes even more reactive to everyday noise.
It’s important to emphasise that hyperacusis is real, valid, and recognised within audiology. It is not “in your head”, and it is not a sign of anything sinister. With the right support, most people can make meaningful improvements in their sound tolerance and overall wellbeing.
How Is Hyperacusis Treated?
One of the biggest challenges is that there is no universally accepted treatment protocol for hyperacusis. Different clinics may use different approaches, and research continues to develop. This is true across UK audiology, NHS pathways, and international research groups.
Even without a single agreed‑upon treatment, several approaches are commonly used and can be helpful:
Education is often the first step. Understanding how the auditory system works — and why sounds may feel overwhelming — can reduce fear and help people feel more in control.
Sound therapy is widely used to help rebuild tolerance. This involves gently reintroducing everyday sounds in a gradual, structured way. It is not about forcing yourself to endure discomfort, but about helping the auditory system settle and respond more normally over time.
For individuals who experience anxiety or fear around sound, psychological support such as Cognitive Behavioural Therapy (CBT) can be valuable. CBT does not treat hyperacusis directly, but it can reduce the distress associated with it and improve coping.
Where hearing loss is present, hearing aids can help restore more natural sound input, which may improve sound tolerance.
A key message — echoed across UK guidance and research groups — is that over‑protecting the ears can worsen hyperacusis. Earplugs should be used for genuinely loud environments, not everyday sound.
